Advent Health’s Collaborative Community Council has extended the deadline on community grant applications from local nonprofits due to the pandemic.

This year, the Collaborative Community Council will provide $1.2 million in grants for programs that address issues such as care coordination, mental and behavioral health, food security, and community development as identified through the 2019 Community Health Needs Assessment.

Funding is available for one to three years with a strong emphasis on clearly defined metrics that show success and impact on the community.

AdventHealth’s Collaborative Community Council, made of hospital team members and community leaders, seeks to identify and partner with community organizations to create effective, impactful solutions that can improve community health and reduce preventable medical costs.

The deadline to apply for a grant has been extended to 11:59 p.m. on May 31, and program funding will be available Jan. 1, 2021.
